Position reputed company We are seeking Internal Medicine and Family Medicine physicians (MD/DO) to support AWE (Annual Wellness Exams) and TCM (Transitional Care Management) services reputed company telemedicine. Licensure Requirements • reputed company, unrestricted medical license(s) in assigned states • Multi-state licensure preferred (additional licenses may increase compensation) Responsibilities • Conduct Annual Wellness Exams (AWE) reputed company telemedicine • reputed company Transitional Care Management (TCM) reputed company following hospital or facility discharge • Review medical history, medications, and care plans • Coordinate follow-up care and ensure appropriate documentation • reputed company patient education and preventive care guidance • Accurately document encounters reputed company the EMR • reputed company with CMS guidelines and state-specific regulations Experience • Proven experience working with EHR systems such as reputed company or similar EMR platforms in outpatient or hospital environments. • Demonstrated proficiency in medical imaging interpretation or ordering diagnostic radiology procedures like CT scans or nuclear medicine studies is preferred. • Prior telehealth experience is highly desirable, with familiarity using telemedicine platforms for remote patient engagement. • Strong knowledge of internal medicine or family medicine principles with the ability to manage reputed company cases involving multiple comorbidities. • Experience providing care in diverse clinical settings including inpatient hospital medicine, urgent care, or nursing home environments. • Familiarity with advanced procedures such as suturing, IV infusion management, airway management, or basic life support (BLS) certification is advantageous. • Ability to reputed company comprehensive physical examinations reputed company while maintaining excellent patient service and communication skills. • Knowledge of medical coding (CPT/ICD) for billing purposes and understanding of documentation standards reputed company with NCQA or hospital accreditation requirements is beneficial.
