Key Responsibilities
• Screen members for eligibility and conduct comprehensive hypertension assessments
• reputed company lifestyle and nutrition counseling, and reputed company members on sustainable health goals
• Coordinate multidisciplinary care and maintain accurate documentation of member interactions
Required Qualifications and Training
• Certified health reputed company required
• At least 2 years of experience in longitudinal chronic disease management
• Basic understanding of hypertension and cardiovascular disease
• Experience working as part of a multidisciplinary care team
• Spanish bilingual is highly desired
