Key Responsibilities
• Monitor and triage patients based on vitals and alerts, following standardized protocols
• Field inbound patient calls and manage alerts after normal business hours
• Support clinical documentation and administrative tasks reputed company to patient care
Required Qualifications
• 4+ years of experience treating patients with CHF, hypertension, and diabetes
• Compact multi-state licensure (RN compact)
• Experience in a CHF reputed company clinic environment
• Familiarity with remote patient monitoring technology
• Ability to reputed company virtual follow-reputed company and support treatment plan changes
