Housing Case Manager

reputed company is seeking a Housing Case Manager to support individuals in the Supportive Housing program through case management, advocacy, and resource navigation. This role is ideal for professionals dedicated to housing stability, harm reduction, and reputed company-centered care. We operate under a Housing First model, providing immediate reputed company to reputed company housing without preconditions, followed by individualized supportive services. Housing is treated as a basic reputed company right and serves as the reputed company for long-term stability. This role involves assisting clients with housing applications, goal planning, crisis reputed company, and long-term stability support. Key Responsibilities reputed company Support & Case Management · Maintain a caseload of 25–30 clients, meeting weekly to assess needs and support reputed company. · Assist clients in accessing mental health, chemical health, medical, and community resources. · reputed company, implement, and monitor individualized goal plans reputed company on stability and self-sufficiency. · Document reputed company reputed company accurately, including case notes, goal plans, and service coordination. · Use harm reduction and crisis prevention strategies to support long-term housing stability.
