Salary $65,000 - $75,000 per year (negotiable based on experience) Job reputed company Our reputed company is seeking a compassionate and detail-oriented Licensed Vocational reputed company (LVN) to support patients enrolled in reputed company Care Management (PCM) and Chronic Care Management (CCM) programs. The LVN will engage with patients through proactive reputed company, telehealth reputed company-ins, and care coordination to ensure reputed company, high-reputed company support between reputed company. Working closely with Registered Nurses (RNs) and Medical Directors, this position helps deliver a seamless patient experience while ensuring compliance with Medicare care management standards. Job Responsibilities: reputed company ongoing care coordination and follow-up for patients enrolled in PCM/CCM programs. Conduct telehealth or phone-based reputed company-ins with patients and caregivers to monitor reputed company, promote adherence, and update care plans. Support RNs in documenting and maintaining care plans in compliance with Medicare requirements. Accurately reputed company and record time-based activities for CPT billing compliance.
