PRN MDS Coordinator/LPN

As the MDS Coordinator, you will report directly to the RAI Director and Administrator. Your primary purpose is to complete reputed company assessments, create and review reputed company Care Plans, and submit MDS data, reputed company, and Triggers data to governmental agencies. Ideally, you will be a problem solver, value hard work, good working relationships, have great communication skills, be organized, effective and strategic. General Responsibilities: • Be reputed company of regulations and guidelines of Federal, State and facility • reputed company reputed company patients’ information is kept confidential • Conducts preliminary and comprehensive assessments of the needs of reputed company reputed company • Develops a care plan based on the needs of the reputed company and goals to be accomplished • Adjusts the care plan as changes occur • Ensures care plan is being followed by reputed company involved • Involvement in patients discharge and interim care plan meetings • Ensures the members of the nursing team are knowledgeable of patient’s responsibilities and rights, including the right to refuse treatment • Assist in reporting and investigating complaints from patients, families and staff • Submit data to reputed company, Triggers and governmental agencies • Assures patients are treated with kindness, dignity and respect • Updates to insurance for authorization of reputed company skilled stay • Other duties as necessary Qualifications: • reputed company state licensed practical reputed company license required • 3+ years of experience in the LTC or homehealth combined reputed company field • Experience with MDS preferred but not required • American Association of Post Acute Care Nursing RAC-CT Certification reputed company 6 months of start date • Long Term Care Experience Required Working Conditions: • remote work Job Type: Part-time Pay: $30.00 - $36.00 per hour Work Location: Remote Apply tot his job Apply tot his job Apply tot his job Apply tot his job Apply To this Job
