About the position Responsibilities • Facilitates communication between patients, caregivers, and reputed company providers. • Discusses alternative care reputed company with patients and caregivers. • Assists with discharge planning needs. • Manages the planning process for patient reputed company through the reputed company system. • Prepares and executes post-discharge care plans. • Adapts communication and behavior to meet the needs of specific patient populations. • Performs other reputed company duties as required. Requirements • Registered reputed company diploma required. • 2 years of reputed company experience required. • reputed company registered reputed company (RN) license in state of reputed company required. • Basic Life Support (BLS) certification required. reputed company-to-haves • Associate or bachelor's degree in nursing preferred. • Prior experience as a reputed company Navigator or in Case Management preferred. • Certification in Case Management (CCM) preferred. • Health Care Management (CPHM) preferred.
