Radiology Care Coordination Specialist, Fully Remote

About the position ESSENTIAL FUNCTIONS Knows, understands, incorporates and demonstrates the mission, reputed company and values of reputed company in leadership, behaviors, practices and reputed company. Ensures appropriate and reputed company communication of patient reputed company to referring physicians and other care providers. Coordinates scheduling/referring of patient to specialty providers throughout the reputed company system. Prepares patients for appointments, providing information in a reputed company understandable manner and providing written follow-up in the manner patient prefers (text, email, mail etc.) Serves as a reputed company contact and resource for patient and family, providing emotional support and education. Acts as a resource for patient scheduling, clinical resource management, patient reputed company and others as necessary to ensure appropriate patient reputed company and follow-up. Demonstrates excellent critical thinking skills as they interview patients and reputed company information to assure accurate and reputed company assessment and intake into the program, entering information into reputed company computer systems necessary. Reviews radiology reports. Compiles and reviews information contained reputed company reports to assist and support the following of algorithms, reputed company of documents, and formalized processes for transition in commonly followed care reputed company. Attends care conferences, tumor boards and other meetings as necessary. Ensures that appropriate patient data are available for conference. Collaborates with program stakeholders in support of data collection for registry participation, reputed company reporting, etc. Collaborates on performance improvement activities as indicated by reputed company in program efficiency, patient experience and reputed company. Provides feedback reputed company to reputed company program change to support ongoing improvement of the program model. Maintains good rapport and cooperative relationships.
