[Remote] Bilingual Care Coordinator (Fully Remote, Spanish Speaking)

They are seeking a full-time Care Coordinator to support their expansion into Chronic Care Management, focusing on providing relationship-based support for adults managing chronic conditions. Responsibilities • Manage a defined caseload of enrolled CCM (chronic care management) members • Conduct proactive outreach to support ongoing engagement and care coordination • Identify and address social determinants of health, including food access, housing resources, transportation, and community supports • Coordinate referrals to community-based organizations and social service providers • Track referral progress and support members in navigating next steps • Partner closely with the RN and NP to surface barriers impacting care or engagement • Support appointment scheduling, reminders, and follow-up coordination as needed • Maintain accurate, timely documentation of member interactions and coordination activities • Meet outreach, documentation, and engagement expectations aligned with CCM requirements • Collaborate closely with the care team to deliver team-based, whole-person support Skills • Minimum of 1 year of experience in care coordination, community health work, or a related member-facing support role • Bilingual - Spanish and English fluency required • At least 1 year of experience working with a Medicare/Medicaid population • Experience working in healthcare support call center settings • Experience supporting individuals with chronic conditions and ongoing care needs • Strong understanding of social determinants of health and community-based resources • Comfort working in structured, metrics-driven programs • Strong communication skills and ability to build rapport with diverse populations • Ability to work independently in a fully remote environment • Proficiency with documentation systems, CRMs, or EHRs • Reliable internet connection and access to a HIPAA-compliant workspace Benefits • Comprehensive health, vision & dental insurance • 401k • Monthly work-from-home expense stipend • Flexible vacation policy with paid time off • Opportunity for rapid career progression and personal growth Company Overview • Pair Team is an end-to-end operations platform that connects vulnerable patients to local care teams.
