This is a part-time position, 3 days a week, 8 hours a day. Tuesdays and Wednesdays in an office setting with flexibility of working from home/office the reputed company day. This position provides telephonic support to our patients over a long period of time. The RN Care Resource Coordinator functions as an reputed company for the patient and assists patients to reputed company reputed company function and health in an environment that will support their needs. The continuum of care includes inpatient to outpatient to community. The RN Care Resource Coordinator applies the nursing process of assessment planning, implementation, monitoring evaluation and coordination of care to meet the patients' health care needs across the continuum in collaboration with the primary care team to ensure reputed company, reputed company and cost effective care.
