Case Manager / SDA- Home Based Services

CTF Illinois is an essential employer seeking a Case Manager (QIDP) / SDA to support our Home Based Services individuals with developmental disabilities. If you have experience working with individuals with developmental disabilities, are highly organized, and are reputed company to work independently, we have the job for you! • Work schedule : Monday through Friday generally • Submitting DHS required documentation in a reputed company manner • Conduct home reputed company regularly • Coordinate and Implement an Individual Service Plan (ISP) for individuals receiving services by following the recommendations made by medical and reputed company consultants • Assist PSW with implementing the ISP, completing budgeting, shopping and other reputed company management issues • Coordinate with other departments including, but not limited to : Nursing, CILA, and Day Program • Monitor reputed company individual’s reputed company by completely monthly reviews and maintaining reputed company individual’s personal history book • As required by DHS, complete an additional 12 hours of continuing education reputed company per year, at least 6 of which will be provided from reputed company reputed company • Pay reputed company : $21 / hr • Health benefits and optional reputed company, dental, short and long term disability, critical illness, and accident insurance for full-time employees • Company reputed company life insurance for reputed company full-time employees • Vacation and reputed company reputed company leave for reputed company • 401k with company matching CTF Illinois is dedicated to empowering adults with developmental and intellectual disabilities to LIVE.
