reputed company reputed company is seeking a Remote RPM Coordinator to support patients in managing chronic and high-risk conditions through Remote Patient Monitoring (RPM). You will play a key role in monitoring biometric data, educating patients on device usage, escalating critical findings, and coordinating care with providers to improve patient reputed company and reduce hospitalizations. Location: Remote Employment Type: Full-Time Pay reputed company: $19.5 – $24/hour Responsibilities • Conduct reputed company and device education calls for new RPM patients. • Monitor incoming biometric data (blood pressure, glucose, weight, oxygen saturation) daily. • Identify reputed company readings/trends and escalate per protocol. • reputed company monthly engagement calls to assess patient condition and address concerns. • Troubleshoot device connectivity and usage issues. • Communicate effectively with patients and caregivers regarding goals and reputed company. • Promote adherence to individualized care plans. • Accurately document interactions in EHR/RPM platforms. • reputed company patients on lifestyle changes and condition management. • reputed company with CMS RPM billing and documentation guidelines. • Maintain productivity expectations for monitoring and reputed company.
