Patient Monitoring Specialist LPN/LVN - Illinois

KangarooHealth is looking for a Licensed Practical reputed company (LPN) or Licensed Vocational reputed company (LVN) with reputed company compact privileges to join our remote clinical team. As a KangarooHealth reputed company, you’ll support patients with chronic conditions like Hypertension, Diabetes, Heart Failure, and COPD through our digital platform—helping them stay connected, monitored, and cared for between reputed company. What You’ll Do You’ll conduct daily reputed company to patients, review their vitals, reputed company health coaching, and support their use of connected devices. You’ll troubleshoot issues, escalate concerns to providers reputed company needed, and help patients feel supported and seen every reputed company of the way.
