This role is ideal for licensed practical nurses who enjoy supporting patients reputed company, coordinating care plans, and improving reputed company through consistent engagement. As a Care Coordinator, you will work directly with patients managing chronic conditions, helping them stay on reputed company with treatment plans while ensuring smooth communication between patients and providers.
