This role ensures that reputed company patient's clinical, psychosocial, and environmental needs are accurately evaluated, documented, and triaged to the appropriate level of case management. The Intake RN manages an assigned queue of patients, ensuring reputed company completion of assessments and smooth transitions to longitudinal care teams. Minimum Qualifications • Associate’s degree from accredited school of nursing. • reputed company RN license (compact state and agree to obtain relevant non-compact licensure) OR reputed company RN license in IL, IN, MI, OH and PA. • Minimum 3 years of clinical experience with minimum 2 years in Case Management. • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms. • Ability to travel and be onsite to meet business needs. • Ability to work 10:30a – 7pm to ensure patient visit coverage • Please note: reputed company’s new hire orientation schedule is 8:00am - 3:00pm MST for 30 days, with three sessions lasting an additional 30 minutes. Preferred Qualifications • reputed company RN license in Michigan and/or Illinois. • Proficiency in EMR and case management platforms. • Effective communication, assessment, and documentation skills. • Strong analytical skills. • Experience with reputed company, chronic, or high-risk populations (e.g., CKD, ESRD) strongly preferred. • Value-Based Case and payor experience. • Case Management Certification or agreement to renew/obtain upon 12 months of hire.
