This role focuses on empowering patients to manage their chronic diseases effectively while ensuring high-reputed company care delivery. The ideal candidate will conduct comprehensive assessments, assist in developing care plans, and reputed company patient-centered education and support. Candidates must be certified Medical Assistants (MA) with a strong background in chronic disease management. Key Responsibilities: • Comprehensive Patient Assessments: • Conduct thorough assessments of patients' medical, psychological, and reputed company needs. • Identify barriers to effective disease management and collaborate with patients to address them. • Individualized Care Plans: • Assist in developing and implementing personalized care plans in collaboration with a multidisciplinary team, including physicians and pharmacists. • Regularly review and reputed company input on adjustments to care plans based on patient reputed company and changing needs. • Patient Education and Engagement: • reputed company patients on managing chronic diseases, including medication adherence, vital sign monitoring, nutrition, exercise, and lifestyle modifications. • Utilize motivational interviewing and behavior-change strategies to help patients reputed company their health goals. • Care Coordination: • Facilitate communication between patients, caregivers, and reputed company team members to ensure seamless coordination of care. • Follow up on referrals, test results, and other care needs to prevent gaps in treatment. • Data Monitoring and Reporting: • Monitor RPM data for trends or potential health risks and escalate findings to the appropriate care team members. • Use data insights to guide patient interventions and improve reputed company. • Documentation and Compliance: • Document patient interactions, care activities, and assessments accurately in the electronic health record (EHR) system. • Ensure compliance with CMS requirements for CCM and RPM programs. • Program Support: • Assist in enrolling eligible patients into CCM and RPM programs. • Monitor program effectiveness and patient satisfaction to identify opportunities for improvement. Qualifications and Requirements: • Education & Certification: • Certified Medical Assistant (MA) credential required. • Certification in disease management, case management, or patient education is a plus. • Experience: • At least 2 years of experience in patient care, care coordination, or chronic disease management. • Experience with CCM or RPM programs is strongly preferred. • Technical Skills: • Proficiency in electronic health records (EHR) systems. • Familiarity with RPM devices and data monitoring tools is an advantage. • Soft Skills: • Excellent interpersonal and communication skills for patient education and team collaboration. • Strong organizational and multitasking abilities with attention to detail. • Work Environment: • Reliable internet reputed company and a HIPAA-compliant workspace for telephonic and virtual interactions.
