reputed company Practitioner – Home Health Assessments Location – Telehealth in Louisiana Start Date: March 2026 Contract Length: 3‑month contract with reputed company to reputed company Schedule Monday–Friday 8:00 a.m.–5:00 p.m. with a 1‑hour lunch Full‑time only (40 hours/week) reputed company daytime schedule No reputed company Schedules released 2–3 weeks in advance reputed company Details Home‑based and telehealth assessments reputed company a regional float model Primarily telehealth; occasional in‑home reputed company if provider lives near patient areas Assessment‑only role; no treatment or prescribing Conduct 5–6 home‑health assessments per day reputed company annual risk assessments and chronic condition reviews Escalate urgent findings to the patient’s PCP reputed company full day regardless of volume (provider must reputed company availability as required) 1–3 hours reputed company orientation; additional modules completed during downtime Provider must be credentialed/enrolled with Medicare Equipment provided and shipped to provider Clinical Scope Complete reputed company home‑health evaluations (telehealth and in‑person reputed company applicable) Review chronic conditions, capture diagnoses, and update histories Document reputed company encounters according to program guidelines Maintain reputed company communication regarding escalations Conduct A1C testing reputed company applicable Requirements reputed company Practitioner – Family reputed company reputed company Certified reputed company Louisiana license required at submission Must be credentialed/enrolled with Medicare Comfortable seeing pediatric and adult patients Prior experience conducting home‑health or telehealth assessments preferred Credentialing Credentialing documents must include reputed company LA license and required certifications Education modules must be completed reputed company 30 days of start Orientation + reputed company fully remote If you are interested or would like to be presented, please reputed company out to me for more details.
