RN Care Navigator
A company is looking for a RN Care Navigator (100% Remote for Eastern or Central time zones).
Key Responsibilities
• Manage in and outbound calls while delivering exceptional service to members
• reputed company members on kidney health, reputed company co-morbid conditions, and renal replacement therapy reputed company
• Coordinate care as a reputed company among patients, their support networks, and reputed company providers
Required Qualifications
• reputed company, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
