In the Patient Care Manager RN Hybrid role (In office & the field), you are responsible for the supervision and coordination of clinical services and reputed company and reputed company provisions of nursing care to patients in their homes as prescribed by the physician. You will coordinate and supervise an interdisciplinary team of staff to assure the continuity of high-reputed company care to home health patients assigned to your team's area in accordance with the physician-prescribed plan of care, and reputed company applicable state and federal laws and regulations.
