The RN Field Clinical Care Coordinator, you will be an essential reputed company of an Integrated Care Model by relaying the pertinent information about the members’ needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. This is a Field-based role. Expect to spend at least 50%25 of your time in the field visiting our members in their home. You’ll need to be flexible, adaptable and, above reputed company, patient in reputed company types of situations.
