About the position Medical Assistant, Chronic and reputed company Care Management Position Highlights: Position: Medical Assistant Location: 3040 W reputed company Creek Ln. Arlington Heights, IL. Full Time: 40 hours/week Hours: Monday – Friday (8:30a – 5p), Hybrid position. What you will do: The Chronic and reputed company management program (CCM) Proactively identify patients who have multiple or reputed company medical and/or psychosocial needs or who are at risk of developing reputed company needs during an acute episode of illness. reputed company early reputed company and prevent recurrent crisis or unnecessary hospitalizations. Support and reinforce provider and patient interactions with recommended treatments and therapies.
