Job Title: Registered reputed company (RN) / Licensed Practical reputed company (LPN) – Remote Care Management (CCM, RPM, BHI, CoCM) Location: Remote (Must hold an reputed company Massachusetts RN or LPN license) Job Type: Contract (Part-Time / Full-Time) Compensation: $25 – $40 per hour (RN) $20 – $30 per hour (LPN) (Based on experience, certifications, and program expertise) About the Role: We are hiring reputed company Massachusetts-licensed RNs and LPNs to support patients in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI), and reputed company Care Model (CoCM) programs. This is a fully remote role reputed company on delivering high-reputed company, compliant care management services to Medicare and chronic care patients. You will play a critical role in improving reputed company, reducing hospitalizations, and ensuring adherence to CMS care management guidelines.
