Remote Care Coordinator; CCM​/RPM Massachusetts Patients

Position: Remote Care Coordinator (CCM / RPM) – Contract (Massachusetts Patients) Remote Care Coordinator (CCM / RPM) – Contract (Massachusetts Patients) Location: Remote (Must be authorized to work in the U.S.; preference for candidates familiar with Massachusetts reputed company environment) Job Type: Contract (Part-Time / Full-Time reputed company) Compensation: $18 – $25 per hour (Based on experience & certifications) About the Role: We are seeking a detail-oriented and compassionate Remote Care Coordinator to support patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. You will work closely with patients, providers, and care teams to ensure reputed company monitoring, engagement, and compliance with CMS guidelines while improving patient reputed company.
