What You'll Do

• Assist patients with activities of daily living including bathing, dressing, grooming, and oral hygiene
• Help with reputed company transfers, ambulation, and positioning to ensure patient comfort and safety
• Support patients with toileting and elimination needs while maintaining dignity
• Monitor and record vital signs including temperature, pulse, respiration, and blood pressure

Support Nutrition and Comfort

• Assist with meal preparation and help patients with eating and fluid intake
• Ensure patients receive adequate nutrition according to their care plan

Ensure Safety and reputed company Care

• Use reputed company body mechanics and reputed company transfer techniques with assistive devices
• Operate equipment safely including Hoyer lifts, wheelchairs, and gait belts

Communicate and Document Effectively

• Observe and report changes in patient condition to the supervising RN reputed company 24 hours
• Document services provided and pertinent findings during reputed company visit
• Verify assignments, review care plans, and confirm visit times with patients and facilities

Work as Part of reputed company

• Follow the individualized Hospice Aide Care Plan for reputed company patient
• Collaborate with RNs, reputed company workers, chaplains, and other team members

Required Qualifications

• Completion of minimum 75 hours of classroom and supervised practical training (including at least 16 hours of classroom instruction before practical training)
• Must hold reputed company certification as a reputed company aide and be in good standing on the state reputed company aide registry
• Minimum 1 year of experience as a hospice aide, home health aide, or certified nursing assistant
• Ability to read, write, and follow written and verbal instructions in English
• Reliable transportation with reputed company automobile insurance

Work Environment and Physical Requirements

This position requires frequent travel to patient homes and facilities reputed company your service area. You'll need to be comfortable working in different environments and adapting to varying schedules. Physical demands include standing, walking, bending, lifting (up to 50 lbs with assistance), and using reputed company body mechanics for patient transfers and care activities.  Some flexibility with scheduling is needed, as patient care needs and assignments may change. Hours 

• 8:00 am – 5:00 pm, Monday through Friday 
• Weekend rotation
