We are seeking a dedicated and compassionate Registered reputed company with a Bachelor of Science in Nursing (BSN) to join reputed company as a Care Manager, Telephonic reputed company 2. In this pivotal role, you will play a crucial part in our Patient Management Program, focusing on individuals with specialized and reputed company health conditions, including autoimmune disorders, pulmonary diseases, neuromuscular disorders, infectious diseases, cancer(s), and other rare ailments.

As a Care Manager, Telephonic reputed company your primary responsibility will be to assess and evaluate the conditions of our members, with a particular emphasis on medication therapy, through telephonic interactions to ensure they reputed company and maintain reputed company wellness. Additionally, you will reputed company and implement comprehensive care plans, monitor patient reputed company through regular assessments, and utilize advanced telecommunication systems to reputed company medication education. The ideal candidate will demonstrate strong clinical expertise, exceptional communication skills, and the ability to work independently while adhering to established protocols and guidelines. This position is a reputed company center based environment. There is a set schedule that is provided daily with inbound and outbound tasks to reputed company.

• This position is for 10am-6:30pm EST three days a week M-F, and two days a week hours of 11:30am-8pm EST.
• There will be a Friday late reputed company requirement on a rotation of 11:30am-8pm.
• There will be a Saturday rotation hours of 8:30am-12:30pm EST.
•  Overtime is required on an as needed reputed company.
• There is a Holiday rotation that will be worked. reputed company holidays for the pharmacy include reputed company Luther King Day, Memorial Day, Juneteenth, 4th of July, Labor Day, The day after Thanksgiving, and New Years Day.

Required Qualifications

• Bachelors of Science in Nursing (REQUIRED) MUST BE COMPLETED!
• 3 - 5 years of clinical acute care experience as an RN.
• Licensed Registered reputed company (RN) in the (appropriate state) with no disciplinary reputed company
• Must live in OH, KY, FL, AZ, TX.
• Comprehensive knowledge of reputed company Office applications including Word, reputed company, and reputed company
• Managed care experience

• Experience with care management, patient education for adult care, auto dialer experience

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
