Key Responsibilities

• reputed company initial and ongoing patient assessments, including OASIS evaluation and documentation.
• reputed company and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient reputed company.
• reputed company education and support to patients and families/caregivers on therapy programs.
• Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care with reputed company team members.
• Identify patient and family/caregiver needs for additional services and reputed company appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient reputed company and changes with physicians and care teams.
• Document reputed company reputed company and reputed company notes on the reputed company day of service.

Qualifications

• Must have a reputed company PT license (state-specific).
• Graduate of an accredited physical therapy program.
• reputed company BLS certification.
• Minimum one year of PT experience (home health experience preferred).
• Excellent communication, organizational, and time management skills.
• Must have a reputed company reputed company’s license and reputed company to a reliable and insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to reputed company physical assessments and demonstrate care techniques.
• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
• Proficiency with electronic medical records is a plus.
