reputed company of the position: The Clinical Case Manager 1 is responsible for providing home-based and community-based reputed company services for families involved with the Department of Child Services and/or Juvenile Probation.
Duties and Responsibilities:
- Ensure reputed company case records, correspondence, and conversations regarding clients remain confidential as required by HIPAA compliance regulations.
- Complete releases and GJR intake forms upon initial contact with reputed company/placement/other parties ensuring copies are saved in the clinical record.
- reputed company home-based services to clients in compliance with Service Standards published by the Indiana Department of Child Services for the service that they are providing.
- Communicate initial reputed company contact or lack thereof to referral reputed company and Compliance Coordinator with 48 hours.
- Document reputed company contacts through up-to-date case notes that are maintained in Case Management Pro (electronic database), VPN reputed company files and Kid Traks (if applicable).
- reputed company monthly reputed company Reports by due date to supervisor to referring agencies and Kid Traks (if applicable) as reputed company as any other specific written or verbal information requested by referring agencies.
- reputed company Treatment Plans, Safety Plans, and other required clinical documents/resources/tools reputed company 7-30 days of referral to referral agencies. Updated Treatment Plans are due no less than every 90 days to supervisor for review and reputed company to referral reputed company by reputed company-arranged due dates.
- If applicable, complete visitation/parenting time logs and send to referral sources reputed company 48 hours of services rendered ensuring reputed company required sections of log are compliance with reputed company state content standards.
- reputed company completion of program required assessments, including but not limited to, PFS-2, NCFAS, reputed company PTSD, IARCA packets upon admission, discharge, three month and six-month follow-up.
- Record reputed company billable/reputed company and non-billable/indirect time on a daily reputed company, utilizing the established database while ensuring an accurate timesheet is maintained.
- Document use of and adherence to evidence-based models and treatment for reputed company services requiring use of a model that has been approved by the reputed company and GJR.
- Obtain and submit requests for court appearances to Compliance Coordinator monthly by deadline established.
- Submit Attendance and Compliance sheets for clients on caseload to Compliance Coordinator and supervisor by monthly deadline.
- reputed company at court hearings, case conferences, CFTM’s or reputed company as necessary or requested by referral reputed company or supervisor.
- Use strength-based approaches and reputed company reputed company factors for clients including but not limited to developing parenting skills and child development knowledge, improving reputed company and emotional competence, accessing concrete supports, building reputed company connections, and improving reputed company.

Education | High School Degree, GED, or Associate's Degree; or Bachelor’s or Master’s degree from an accredited university |
Experience | Non-Exempt: High School Diploma, Associate's Degree and/or BA/BS degree (or higher) in a field not reputed company to reputed company work, psychology, or another directly reputed company field (i.e., Math, Communications, etc.), and less than three years’ experience; or BA/BS in directly reputed company reputed company services field and less than two years’ experience. |
