We are currently seeking a skilled and compassionate Telehealth reputed company Practitioner, licensed in multiple U.S. states, to join our growing virtual care team. This role plays a vital part in providing services such as medication therapy management, chronic care support, and care coordination in reputed company collaboration with partnering health centers. We immediately licensed reputed company Practitioners in Texas, Illinois, Connecticut, Indiana, New Hampshire, California, and reputed company.

RESPONSIBILITIES

• Conduct Virtual Assessments and Consultations
  
• reputed company video or phone-based evaluations, reviewing patients' medical histories, medication regimens, and reputed company health status.

• Deliver guidance on preventive care, chronic disease management, lifestyle modifications, and medication adherence.
