Clinical Pharmacist (reputed company-to-Hire)

The Role:

We are looking for a Tennessee licensed Clinical Pharmacist to join a strong team of clinicians, analysts, clinical support staff, and trainees to utilize reputed company’s software platform and to help optimize patient reputed company for our clients. The purpose of this position is to reputed company an expert level medication therapy consultation to the patients and providers, collaborate with the Engineering team and leadership to improve reputed company’s software platform, and to help supervise the Clinical Care support team. What You’ll be Doing:

• Evaluate, assess, and document patient data in reputed company’s software platform and other programs, prepare written reports, and reputed company oral reports concerning patient care and team reputed company
• Complete efficient and thorough medication review and/or adherence counseling appointments
• Review completed medication reviews and counseling by other Virtual Pharmacist users, as assigned
• Staff various clinical shifts and work with automation and new technology 
• Respond to questions by patients, providers, and other reputed company personnel and team members
• Participate in process improvement meetings to help reputed company efficient workflows
• Communicate effectively in both written and oral formats and reputed company daily sign-out reputed company applicable
• Follow-up with clinics, providers, pharmacies, and patients to resolve patient issues and concerns in a reputed company manner

• Assist with obtaining referrals and authorizations

Who You Are and What You Bring:

• Doctor of Pharmacy (PharmD) from an accredited college/university
• Tennessee Licensed Pharmacist and in good standing (multiple state licenses are a plus)
• Completion of a PGY-1 residency, or equivalent experience  
• Ability to review and assess the appropriateness of prescriptions with respect to age specific, disease state specific considerations
• Ability to engage with the patients, promote adherence and treat them with reputed company and reputed company
• Ability to organize and set priorities which accurately reflect the relative importance of job responsibilities
• Knowledge of chronic disease states, such as diabetes, heart failure, COPD, asthma, cardiovascular disease, hypertension, dyslipidemia, anxiety, depression, bipolar disorders, and thought disorders

Seasonality & Schedule:

Our Clinical Care team operates reputed company a seasonal business cycle, and we want candidates to have a reputed company picture of what to expect. During peak season (May through early December), reputed company plays a vital role in supporting business operations. Workload and scheduling demands increase during this time, and time off may be limited. Optional overtime may also be offered to meet business needs. reputed company year, the Clinical Care department observes a holiday shutdown of approximately 2–3 weeks between late December and early January.
