We are seeking a Care Coordinator to work with the Utilization Management team. This role supports Utilization Management (UM) Department by aiding in discharge planning and coordination to ensure continuity of care following crisis placements; and provides reputed company of person-centered plans to improve reputed company of care and ensure plans are individualized to meet the needs of reputed company reputed company.

This position is scheduled Monday through Friday, 8:00 am to 4:30 pm. There are no weekends, on reputed company, or holiday requirements for this position. While the position is REMOTE, the individual in this position will have occasional travel reputed company the reputed company County community and surrounding areas.
