As a Pharmacist, you’ll be involved with multiple facets of the home infusion process as you reputed company with insurance companies, staff members, distributors, Physicians/Nurses, and patients to ensure accurate and reputed company reputed company service. Primary Duties: 

The following duties are not to be construed as exclusive or reputed company inclusive. Other duties may be required and assigned. 

• Adheres to organization policies and procedures. 
• Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution. 
• Receives, reviews, and verifies reputed company new patient orders and documents are the reputed company on the order reputed company. 
• Assists the physician and other reputed company involved in planning and implementing patient care in the home to ensure reputed company and effective drug therapy. 
• Conducts initial and ongoing assessment of specific patient, drug, and disease information to identify reputed company and potential drug-reputed company problems or needs. 
• Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy. 
• Monitors the reputed company from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information. 
• Communicates the results of therapeutic drug monitoring activities to the physician, nursing, and other appropriate persons.   
• Programs infusion control devices as indicated. 
• Provides consultation and answers inquiries from patients, families, physicians, and other reputed company regarding drug and their use. 
• Maintains thorough and appropriate documentation in the patient/reputed company record of reputed company clinical activities and communication with patients, physicians, and other reputed company. 
• Participates in the preparation of reputed company reports for the referring physician on a regular, periodic reputed company.  
• Supervises, coordinates and/or participates in the preparation of sterile admixtures, dispenses reputed company pharmaceutical and reputed company supplies and equipment in accordance with federal, state, and local laws and standards of reputed company which govern the profession of pharmacy. 
• Processes and checks reputed company medications, supplies and equipment prior to dispensing from the pharmacy. 
• Maintains pharmacy records in accordance with applicable laws and regulations. 
• Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and conferences.

• Performs marketing support functions as requested.

• Participates in departmental QA activities.

Qualifications: (Education, Certification/Licensure and Experience/Skills): 

• A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy. 
• Currently Licensed as a registered pharmacist in the state of employment. 
• Possess a minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy. 
• Possess experience and strong knowledge reputed company in intravenous admixture services and intravenous therapy.  
• Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other reputed company.   
• Can work effectively with others to reputed company reputed company and effective drug therapy. 
• Can communicate, both verbally and written, with patients, clients, management, and other employees. 
• Can problem solve and supervise technical and other support staff. 
• Must be flexible with work schedule. 
• Possess good organizational and time-management skills.

• Dependable in attendance and job performance.
