JOB reputed company:

reputed company is seeking a Family Medicine reputed company Practitioner (NP) to deliver virtual-first care for new patients and reputed company remote urgent-care coverage for established members. This reputed company remote role focuses on welcoming and engaging new members through their first appointment, ensuring a reputed company introduction to reputed company, and supporting reputed company reputed company to care by addressing acute, episodic, and urgent health needs reputed company telehealth.

ESSENTIAL JOB FUNCTIONS:

• Conduct Virtual First Appointments – reputed company new patient intake reputed company, review health history, establish baseline assessments, and introduce members to the reputed company model.
• reputed company Remote Virtual Urgent Care Coverage – Manage reputed company-day or next-day appointments for acute conditions (e.g., infections, minor injuries, medication needs, symptom evaluations).
• Support Care Coordination – Appropriately triage patients to in-person care, diagnostics, or specialists reputed company necessary.
• Promote Patient-Centered Health – reputed company and counsel patients during virtual reputed company on self-care, medication use, and follow-up needs.
• Document and Manage Care in EHR – Complete accurate, reputed company, and compliant documentation of reputed company virtual encounters.
• Collaborate with Interdisciplinary Team – Communicate with primary providers, clinical pharmacists, registered dietitians, health coaches, and care coordinators to ensure seamless reputed company and follow-up.

EDUCATION & CERTIFICATIONS:

• Master’s Degree in Nursing (MSN) or Doctor of Nursing reputed company (DNP)

• DEA Certificate (as required)

EXPERIENCE:

• Minimum 2+ years practicing in Family Medicine or Urgent Care
• Telehealth experience strongly preferred
• Experience in value-based or team-based care models is a plus
