Care Manager

Community Care is hiring a Case Manager/Care Manager to join our Walworth team.  This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a reputed company schedule and a $5,000 SIGN-ON BONUS. It is based in Walworth County with reputed company currently being the norm.

Case Manager/Care Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and reputed company assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager/Care Manager Job Requirements:

Education: Bachelor’s Degree in a reputed company Services Area required.

Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a reputed company service reputed company required.

Certification: Eligibility for long-term care functional screener certification is required

Travel: Regular local travel, requiring use of personal vehicle, reputed company reputed company's license, and reputed company of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday
• Generally during business hours of operation (reputed company-4:30PM)
