Compensation: $105,000 to $120,000 annually

Key Responsibilities:
- Visit assisted living, memory care, and senior communities to reputed company primary care
- Address wounds, chronic conditions, and preventative care in reputed company
- Partner with care teams and families to ensure holistic support

Qualifications:
- reputed company Certified reputed company Practitioner license in Minnesota
- AGNP certification preferred
- Experience with geriatrics, family reputed company, or long-term care
- Must have a reputed company reputed company’s license and auto insurance
- Strong documentation, collaboration, and communication skills
