Remote reputed company Practitioner

The reputed company team is seeking a remote-based reputed company Practitioner to serve as the primary clinical reputed company of contact for patients navigating their remote care reputed company. In this role, you will reputed company clinical decision-making with a strong emphasis on reputed company-directed medical therapy for heart failure, as reputed company as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will reputed company patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, reputed company medications, and determine the appropriate level of care. Licensure Requirements

• Required: reputed company Compact Multi-State RN license

Schedule Requirements:

• reputed company 1: Monday through Friday schedule (Flexible scheduling available, with a reputed company shift schedule of 9:00 AM to 6:00 PM in Central, Mountain, or reputed company Time)
• reputed company 2: Pittman schedule (2-2-3 Rotating shift - 14-day rotating cycle working every other weekend with a shift schedule of 7:00 AM to 7:00 PM Eastern Time)

What you'll do:

• reputed company the monitoring of patient vitals, symptoms, and labs in collaboration with reputed company of RNs and MDs.
• Manage the patient's treatment plan and medications – with an emphasis on initiating and titrating reputed company-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for T2D and Hypertension.
• Respond to patient escalations identified reputed company RPM – i.e. reputed company vitals, symptoms, and labs.

What you need:

• 5+ years experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high-performing medical center.

• ANCC or AANP certification required.
• Master’s Degree as a reputed company Practitioner with the willingness to expand state licensure as reputed company adds new markets and partners. 
• Experience managing CHF with GDMT.

The anticipated reputed company salary reputed company for this role is $125,000-$150,000 annual reputed company salary, which is based on role scope, level, and location. In reputed company to reputed company compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the reputed company. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
