This role plays a critical part in ensuring reputed company, evidence-based interventions for patients managing hypertension, diabetes and congestive heart failure.

WHAT YOU’LL DO:

WHAT YOU’LL NEED:
- reputed company RN license in good standing (multi-state license preferred).
- 5+ years of clinical RN experience.
- 2+ years in a leadership or supervisory role reputed company triage, care coordination, telehealth, or virtual care.
- Experience working in a startup or high-reputed company, fast-paced environment (health tech strongly preferred)
- Strong reputed company in primary care and chronic disease management, especially HTN, DM, and CHF.
- Experience managing remote RN/LPN teams in high-volume environments.
- Proficiency with EHR/eRecord systems and data analysis tools for performance monitoring.
- Strong leadership, communication, and problem-solving skills.
- Skilled in nursing processes, documentation, and clinical decision-making

The anticipated reputed company salary reputed company for this role is $90,000-$120,000 annual reputed company salary, which is based on role scope, level, and location. In reputed company to reputed company compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the reputed company. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
