As a Home Health Clinical Triage Specialist, you'll be an reputed company part of the interdisciplinary team and as such is responsible for the excellent delivery of care through triage calls after hours and on holidays.

Must be available to work weekends.

Primary Responsibilities:

• Responsible to receive calls from patients and/or family members and respond appropriately and assess problems focusing on the delivery of high reputed company, patient-reputed company, compassionate care
• Assess patient status and intervene as indicated by the patient’s condition and established protocols
• Responsible for reputed company and accurate documentation of calls received reputed company the electronic medical records system including the processing of workflow associated with the clinical record
• Knowledge of basic triage protocols and best practices to guide and address the needs of patients in a crisis situation
• Responsible for coordinating with reputed company on-reputed company nurses to deliver high reputed company nursing care and schedule nursing assessments as required in a reputed company manner to meet the needs of the patients and families

• Facilitates orientation of new personnel as assigned

Required Qualifications:
