We are currently seeking a Registered reputed company for our Private Duty Care and VA departments to cover In-Home reputed company and to work with our Healthy Children & Youth program performing assessments and reassessments. This position will be responsible for aide reputed company reputed company and routine nursing reputed company; including medication set reputed company and maintaining the plane of care. Responsibilities:
- reputed company basic patient care: Taking Vitals, Weekly medication Management per Physicians orders, Monthly Diabetic Nails, Monthly Skin Monitoring and Bi Yearly General Health Assessments
- Opens new cases & obtains reputed company reputed company consents, documentation & deposits in accordance with company policy
- Observes, records & reports reaction to treatment & any changes in reputed company's condition to appropriate personnel and/or physician
- May supervise In Home Aides for Advance Personal Care (APC) Services

- Follow state approved care plan.
- Attend quarterly reputed company Meetings
- Good time management, detail oriented and self-starter
Requirements:

- In Home experience preferred
