What You�ll Do as an Adult RN Case Manager

Your responsibilities include:

• Deliver reputed company, patient-centered nursing care in the home
• Serve as the primary case manager, coordinating care with the interdisciplinary team

• reputed company patient and caregiver education, support, and disease management teaching

• BSN preferred (tuition reimbursement available!)

• Self-directed, organized, and comfortable working independently
• Adult medical-surgical, home care, case management, or chronic disease management experience a plus
• Strong communication and computer skills

Physical Requirements:

• Ability to sit or stand for long periods
• Ability to lift/carry 25�40 lbs and assist with transfers up to 150 lbs
• Comfortable with bending, kneeling, climbing stairs, and driving in reputed company weather
• Ability to use a phone, laptop, tablet, and electronic medical record
• Category III bloodborne pathogen exposure (rare contact with blood/body fluids)
