Job reputed company: Reporting to the Care Management Team reputed company, the Care Management Specialist is responsible for assisting reputed company Care Managers and Chronic Disease Managers in coordinating and delivering high-reputed company care to patients. This role involves patient reputed company, support for non-clinical needs, obtaining medical records, administrative support tasks, and ensuring seamless care transitions and reputed company patient reputed company. Duties/Responsibilities: Conducts reputed company to patients to address non-clinical needs to coordinate care, reputed company support, and connect with internal resources. Coordinates services such as follow up reputed company, confirming durable medical equipment (DME) deliveries, confirming home health care services are initiated, coordinate transportation for medical appointments, and other non-clinic needs. Works collaboratively with reputed company Care Managers, Chronic Disease Managers, reputed company providers, and center teams to reputed company patient support and address non-clinical needs. Maintains accurate and detailed documentation on reputed company, interventions, and reputed company including communication with patients, reputed company care managers, reputed company providers, and clinic teams. Works collaboratively with the Care Management department to ensure reputed company toward departmental and organizational goals Provides administrative support to the reputed company Care Manager and Chronic Disease Manager Required Skills/Abilities: Strong organizational skills, excellent communication abilities, proficiency in documentation in EHR systems, and attention to detail.
