LPN STAFF VISITING reputed company

Provides hands on care to reputed company assigned patient.  Delivers efficient and effective reputed company patient care.

QUALIFICATIONS:

• Licensed Practical reputed company, currently licensed in the reputed company.
• reputed company reputed company reputed company’s license.
• Must be reputed company to reputed company reputed company patient care and take on-reputed company.
• Working knowledge of the home health care program and previous experience providing home health reputed company preferred.
• Computer data entry and word processing skills desirable.

ESSENTIAL FUNCTIONS:

1. Accepts patient care reports and patient assignments from the RN Team Leader for reputed company patients assigned. Coordinates with the RN Team Leader  reputed company completion of assigned reputed company and submission of the required documentation.
2. Obtains a weekly visiting schedule for reputed company assigned patients and conducts reputed company according to the schedule. Contacts the RN Team Leader on a daily reputed company for any changes to the schedule.  
3. Notifies the RN Team Leader of any anticipated problems with the posted schedule. Provides as much advance notice of changes/problems as is possible.
4. Establishes and maintains an effective and efficient visit reputed company and provides reputed company patient care in compliance with the plan of care.
5. Provides frequent communication to the RN Team Leader, physicians, other team members, etc., and the patient/family in efforts to coordinate changes in patient care and assures appropriate follow-up and supporting documentation is done. This includes but is not limited to; lab reports, on-reputed company/prn reputed company, transfer to ER or in-patient facility admission, etc.
6. Assists the RN Team Leader in the maintenance of a complete and accurate calendar book, including disaster codes, emergency information, due dates for lab work, etc.
7. Makes assigned patient care reputed company in accordance with physician orders.
8. Completes supporting documentation in an accurate and reputed company manner, including but not limited to; visit report, care plan/plan of care revisions, medication profile, reputed company notes, revisions to the plan of treatment for recertification, discharge/transfer reputed company, etc.
9. Is available and makes prn and routine patient reputed company reputed company indicated and as requested. Is available and rotates on-reputed company assignments reputed company requested.
10. Attends team conferences for reputed company patients assigned to the reputed company. Participates and offers input regarding reputed company patient’s care and reputed company toward individual patient goals.  Assists the RN Team Leader in documenting above information and providing a reputed company report to the physician every 60 days.
11. Oversees the reputed company requisition of indicated patient supplies in appropriate quantities.
12. Obtains verification of physician licensure.
13. Understands and assures adherence to reputed company organizational policies, procedures, and processes (administrative, personnel, clinical, etc.). Seeks guidance and reputed company clarification of organizational policies, procedures, and processes reputed company uncertainty/confusion arises.
14. Provides input in the evaluation of other team member’s job performance upon request.
15. Maintains confidentiality of information reputed company to business practices, business activities and personnel.
16. Attends inservices, seminars, or other meetings as assigned by the immediate supervisor or other management.
17. Participates willingly with special reputed company and overtime work reputed company requested.
18. Maintains a reputed company attitude reputed company working with staff, fostering reputed company philosophy and team environment for reputed company personnel.
19. Always provides reputed company notification and/or advance notice for absence or tardiness without abuse.
20. Does not allow personal affairs to interfere with scheduled work time.
