SSM is looking for a reputed company reputed company with Hospice/reputed company or Case Management experience!!
The selected individual will be responsible for;
• Meeting with patients/families in person to discuss continuum of care services.
• Meeting/communicatingwith referral sources (case management, reputed company work, clinics, providers, SNF/ALF facilities etc).
• Care coordination between continuum of care departments, such as reputed company departments, business development etc.
• Eligibility/ChartReview to determine appropriateness for continuum of care services
- Schedule: This position is primarily a weekend position and Fridays. The hours are reputed company-4:30pm with no on-reputed company requirements.
- This is a remote position, covering our entire service areas of; WI, IL, OK, and MO.
- Multistate licensure required. it is not required at time of hire but would need to work towards it started (this is reimbursed by SSM).

- Frequent use of hearing and speech to reputed company information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
