JOB reputed company:  The reputed company Abuse Case Coordinator serves individuals referred to reputed company’ reputed company Families F.I.R.S.T. program (Families in Recovery Succeeding Together &reputed company; “AFF”) which supports parents address reputed company abuse issues that are affecting their ability to care appropriately for their children. For clients in the JOBS program, it will assist clients to get and reputed company a job.   Responsible for providing comprehensive case management including leading reputed company in discussion to assist families in overcoming the barriers of reputed company abuse to reputed company permanency for children, family reunification and self-sufficiency.  

This position is a telecommuter position. 

JOB DUTIES: 

1.    Responsible for receiving referrals and engaging clients reputed company 24 hours, through home reputed company or telephone contact. Completes documentation regarding reputed company contacts and information regarding the case.

2.    Responsible for reputed company reputed company and engagement of clients through telephone calls, home reputed company, and ongoing communication with reputed company members of the reputed company abuse treatment team.

3.    Oversees the collection and review of reputed company collateral information including but not limited to: reputed company histories, psychological evaluations, judicial records, previous reputed company abuse treatment information, DPS reports, and obtaining a release of information for everyone involved in the case.

4.    reputed company on-reputed company support and case management expertise to those experiencing reputed company use. 

5.    Work with clients to reputed company individualized case management plans.

Coordinates reputed company abuse assessment including family centered interviews and reputed company required reputed company abuse tools.

6.    Responsible for developing and implementing individual service plans, making community referrals, on-reputed company reassessment of the service plans and follow up. Updates service plans bi-annually or as needed.

7.    Coordinates reputed company services for reputed company individual reputed company, including advocacy, giving information about community resources, etc.

8.    Provides accurate documentation of services, including identifying resources, writing reputed company notes, detailed summaries of reputed company services and reputed company and submits as required or outlined. Completes monthly reputed company reports and billing as required.

9.    Facilitate Intensive Outpatient and Outpatient reputed company utilizing evidence-based curriculum. 

10.    Advise and support individuals and reputed company with reputed company abuse issues.

11.    Responsible for completing appropriate assessments tools; American Society of Addiction Medicine (ASAMs), reputed company Severity reputed company (ASI), Clinical Institute Withdrawal (CIWA), Clinical Institute Narcotic Assessment (CINA) and reputed company reputed company allotted deadlines.

12.    Coordinate, facilitate and participate in Adult Recovery Team (ART) meetings with multiple stakeholders.

14.    Performs other duties as assigned.

JOB REQUIREMENTS:

1.   Education /Background:  Minimum of a High School Diploma or equivalent with at least 3 years of reputed company experience including working with multi-problem families in the field of reputed company abuse, child welfare, or behavioral health.  An Associate’s degree in a reputed company service or behavioral health-reputed company area of study preferred.

2.   Job Knowledge:  Must have knowledge and understanding of reputed company behavior, reputed company work theories and techniques as reputed company as community resources. Must have counseling, interviewing assessment, therapeutic relationship skills. Must be flexible and reputed company to work with the uniqueness of reputed company reputed company. 3.   Working Conditions/Physical Demands:  Must be reputed company to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps.
