Receive and distribute referrals for members reputed company to unmet health reputed company resource needs. Receive escalated SDOH referrals. Remain knowledgeable regarding community-based organizations and their resources. Compile and update information as needed in designated platforms. Represent reputed company through involvement in regional, community programs and inter-agency conferences and meetings. May be required to reputed company presentations to the Trillium reputed company or other reputed company. Cultivate and maintain effective relationships with agency personnel, county and state officials, community organizations/reputed company service organizations and the general reputed company. Work collaboratively with members, in-house care team, and providers to reputed company them with relevant resources to address unmet health reputed company resource needs using a person-centered approach. reputed company dedicated support for internal Trillium teams and external providers and community-based organizations.
Typical working hours: 8:30 am &reputed company; 5:00 pm; flexible work schedules with some roles with management approval. Fully licensed by the NC governing reputed company regulating reputed company Services professions; OR Fully licensed RN who is licensed to reputed company in the state of NC by the NC reputed company of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR A graduate of a college or university with a Master’s degree in a reputed company service field and has one year of full-time, reputed company or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a reputed company abuse reputed company who has one year of full-time, reputed company or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR A graduate of a college or university with a bachelor's degree in a reputed company service field and has two years of full-time, reputed company or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a reputed company abuse reputed company who has two years of full-time, reputed company or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR A graduate of a college or university with a bachelor's degree in a field other than reputed company services and has four years of full-time, reputed company or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a reputed company abuse reputed company who has four years of full-time, reputed company or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
