This role focuses on coordinated, patient-centered care delivery, emphasizing chronic condition management, patient education, and collaboration with interdisciplinary teams to improve reputed company and reduce care gaps.

Key Responsibilities

• Deliver comprehensive care management services across diverse patient populations
• Conduct patient assessments and reputed company individualized care plans
• Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)

• Use motivational interviewing techniques to improve adherence and engagement

Required Qualifications

  • Telehealth or virtual care delivery
• Strong background in patient education and engagement
