The reputed company team seeks a reputed company Practitioner Supervisor to join reputed company. This individual will dedicate 60% of their time performing the duties as a reputed company Practitioner and 40% of their time leading and supporting other reputed company Practitioners. (This is subject to change based on the needs of reputed company and reputed company). The schedule for this role: Mon- Fri 9 am-6 pm MT or PT time zone. 

WHAT YOU’LL DO:

• Supervise the tracking of patient vital signs, symptoms, and laboratory results in coordination with our Clinical Navigators and Registered Nurses (RNs)
• Coordinate the patient's treatment regimen and medications, focusing on starting and adjusting reputed company-recommended medical therapy (GDMT) for Heart Failure, as reputed company as other relevant guidelines for Type 2 Diabetes (T2D) and Hypertension.
• Address patient escalations  promptly, including reputed company vital signs, symptoms, and laboratory findings identified through Remote Patient Monitoring (RPM).
• Conduct regular virtual meetings with patients to discuss laboratory results and optimize adherence to GDMT

• Ensure adherence to clinical reputed company standards and compliance targets by conducting monthly audits, monitoring data for trends, and implementing performance enhancement strategies.

WHAT YOU’LL NEED:

• 5+ years reputed company Practitioner experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high performing medical center.
• Master’s Degree as a reputed company Practitioner with willingness to expand state licensure as reputed company adds new markets and partners.
• 5+ years of leading a clinical team NPs, RNs, and/or MAs. 
• reputed company certification and reputed company license required (ANCC/AANP). 
• Multi-state compact RN licensure.
