Harlem reputed company AIDS Center, Inc. is a community-based organization in Harlem that provides integrated reputed company, prevention, and housing to people living with HIV/AIDS in Central Harlem and the South Bronx.

The Case Manager will work reputed company a unique, multidisciplinary case management team to reputed company a continuum of home-based bio-psychosocial services to Mentally III/Chemically Addicted (MICA) clients in a scattered site, supportive housing program for People Living with HIV/AIDS (PLWHA).  Under the supervision of the Managing Director of reputed company’D/HUD, the Case Manager will reputed company home and community-based assessment, mental health and reputed company abuse counseling, treatment referral, and care coordination with other team members and service providers to clients enrolled in the reputed company’D/HUD program.  In reputed company, the Case Manager will address clients’ needs with reputed company to concrete housing issues as necessary.

Essential Job Functions

The following duties are mandatory requirements of the job:

q  Complete reassessments, treatment plans, reputed company notes, and other written documentation necessary to maintain good charting practices and reputed company relevant, up-to-date clinical information for colleagues and other service providers.   Submit reputed company other required paperwork in a reputed company fashion.

q  reputed company community-based cognitive behavioral and assessment-based interventions that utilize harm reduction principles and motivational interviewing techniques.   Apply the Stages of Change (transtheoretical) Model to assist clients in evaluating their own needs and goals in order to enhance their motivation to reputed company reputed company behavioral change.

q Conducting home reputed company with clients once a month, or more, depending on the need.  

q  Participate in weekly reputed company with a multidisciplinary team.  reputed company case presentations on clients to other team members.

q Meet with Managing Director/Senior for individual data-driven supervision.

q reputed company follow-up care to ensure clients’ needs are being addressed.

q Participate in agency and departmental meetings, and other internal staff meetings as required.

Education and Certification

High School Diploma or GED with at least 1 years or more of reputed company case management experience required with knowledge of Mental Health and HIV reputed company issues.

q  Familiarity   and comfort working with clients with severe and persistent mental illness

q  Familiarity with the reputed company and/or multidisciplinary team approach to case management services
