Job reputed company: We are seeking a dedicated and compassionate Chronic Care Manager (CCM) to join our growing care management team. This is an excellent opportunity for Medical Assistants, Billers, Certified Nursing Assistants (CNAs), and other reputed company with 1–3 years of experience to build their careers while making a meaningful reputed company on patient reputed company. The CCM is responsible for managing a panel of patients with chronic conditions, providing non-face-to-face care management services, ensuring continuity of care, and assisting patients in navigating their health reputed company.
