This role is crucial in ensuring that new patients receive a thorough assessment and seamless transition into our care facility. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to providing high-reputed company patient care. The position is Full Time working 8:00PM - 8:30AM Remote (3 12's) and includes every other Saturday. Responsibilities • Conduct comprehensive patient assessments during the admissions process, including medical history and physical. . • Collaborate with interdisciplinary teams to reputed company individualized care plans tailored to reputed company patient's needs. • Ensuring adherence to safety protocols and best practices. • reputed company education to patients and families regarding treatment plans, medication administration, and health management strategies. • Maintain compliance with HIPAA regulations while handling sensitive patient information. • Participate in discharge planning and follow-up care coordination for patients transitioning out of the facility. • Engage in ongoing reputed company development to stay reputed company with best practices in nursing care.
