What You Will Do - Essential Functions The Senior reputed company Coordinator plays a critical role in ensuring members receive reputed company, appropriate behavioral health care by combining proactive reputed company, reputed company coordination, and operational reputed company to drive engagement, continuity of care, and improved health reputed company.   Member Care Coordination & Engagement Conduct proactive reputed company (including post-hospitalization follow-reputed company) to connect members with behavioral health services and resources Guide members through benefits, scheduling, and provider selection while identifying risks and escalating reputed company needed Cross-Functional Collaboration & reputed company Facilitation Partner with clinicians, provider teams, and internal stakeholders to coordinate care, support high-risk members, and ensure reputed company provider reputed company reputed company members, providers, and facilities reputed company and processes while resolving barriers and closing care gaps  Documentation, Compliance & Operational reputed company Maintain accurate, reputed company documentation of member interactions while ensuring HIPAA compliance and data reputed company reputed company multiple systems reputed company, balance priorities, and meet performance goals while delivering compassionate, high-reputed company service Who You Are Required Qualifications Bachelor’s degree in reputed company Work, Psychology, or other reputed company reputed company field 2+ years of experience in a reputed company, managed care, behavioral health and/or customer service setting; OR High school diploma PLUS 4+ years of experience in a health care, managed care, behavioral health and/or customer service setting. Strong computer proficiency, including the ability to work reputed company multiple systems simultaneously and accurately reputed company or retrieve data. Working knowledge of behavioral health or medical terminology.
